The DOT Physical Exam Process
Our Lancaster office streamlines DOT physicals so you won't have unneeded delays in getting your DOT medical card. All of our providers are federal certified through Federal Motor Carrier Safety Association to perform DOT physicals.
Step #1: The Paperwork
You’ll begin by completing the first two pages of the Medical Examination Report Form (aka the “long form”). This should take no more than five minutes. It’s here that you’ll be asked things like:

>Have you ever had any surgeries?
>Are you currently taking any medications?
>Do you have any cardiovascular problems, kidney issues, hypertension, diabetes, sleep apnea, etc…?
Step #2: The DOT Medical Exam
The exam portion is where the doctor will review and clarify any of your answers to the paperwork portion that you completed. Then, he will conduct a brief basic physical exam.
The exam lasts less than 20 minutes and includes things like..
>Urinalysis: You will be asked to provide a urine sample. THIS IS NOT A DRUG TEST. Instead it’s a screen for uncontrolled diabetes and underlyering kidney issues.
>A check of vitals including blood pressure and vision and hearing tests.
>General Systems Review: Eyes, Ears, Cardiovascular, Lungs and Neuromusculoskeletal System.
Step #3: The DOT Medical Card
Once your exam is completed and meets the requirements of the FMSCA, you will receive your DOT medical card.
The DOT card will have all the information you need for the following licenses:
>Commercial driver’s license (CDL) for any state.
>Hoisting Engineer License
>School Bus Driver License

To avoid any delay in receiving a DOT medical card, you’ll want to bring a few things to your appointment, including (if applicable):
>Driver’s license (or other form of photo ID)
>Glasses and/or contacts
>Hearing aids
>A full bladder (for the basic urinalysis – not a drug test)
>Medication list
>Most recent Ha1C (if a diabetic)
>Form MCSA-5870 if an insulin-dependent diabetic
>CPAP compliance log (if diagnosed with sleep apnea)
>Additional paperwork/medical clearances may be needed if you have a history of heart disease, stroke, seizures or other neurological disorders). -
No. No DOT physical includes a drug test. The urinalysis that is a part of the DOT exam i strictly looking for the presence of any sugar, blood of protein in one’s urine. The urinalysis is used to identify any potential uncontrolled diabetes or kidney conditions that may be underlying and affect the driver’s safety.

No. Insurance does not cover DOT physicals

In Lancaster (and all of Pennsylvania) a DOT medical card can be valid for up to a maximum of two years. Some medical conditions do restrict a medical card to less time. The most common reasons for a reduced medical card are hypertension, diabetes, obstructive sleep apnea and heart disease.
It’s imperative to renew a DOT medical card before it expires or one risks hefty fines and/or getting a vehicle “pulled off the road.” Lancaster CDL holders also risk getting their commercial license downgraded to a Class D driver’s license with a lapsed medical card.

Yes – certain medical conditions such as uncontrolled hypertension, uncontrolled sleep apnea, uncontrolled diabetes and certain cardiovascular conditons may prevent a driver from passing the DOT.
In most cases, these conditions can be controlled with medications or a CPAP (for sleep apnea) and would them not pose a problem for one passing the physical.

No. Only providers who are certified medical examiners through the Federal Motor Carrier Safety Association are authorized to perform DOT physicals.
All providers at Keystone DOT Physicals are federally authorized to perform physicals to any Pennsylvanian or residents of any other state in the US.

If you hold a category A or C commercial driver’s license in Lancaster, PA, you are also required to submit a copy of your valid DOT medical card to the Pennsylvania DMV. This process is commonly referred to as self-certification and can be completed online or via mail every two years.
CDL holders can self-certify either online using Pennsylvania’s CDL Self Certification System or in person at any Pennsylvania Drivers License Issuance Office. Mailed, faxed or emailed self-certifications are not accepted.
